"I Will Follow You Into the Dark" is the seventeenth episode of the fifth season of the ABC series, Grey's Anatomy. It aired on March 12, 2009.

Summary

Derek has been sitting on the couch eating and lounging for days, depressed by the death of his last patient and the lawsuit that followed.

Cristina accidentally scares Owen while he is sleeping in the hallway on a gurney causing him to push her into a cart, cutting her arm. She tells Meredith it was an accident, but Owen seems visibly distraught by the incident.

A family comes in who are carrying a gene which puts them at a huge risk to develop cancer. They want Dr. Bailey to cut their stomachs out to reduce their risk of getting cancer.

A teenager comes in through trauma with a history of grand mal seizures. She had a seizure during a performance. She's in V-Fib but cannot be shocked without risking killing her. Owen decides to shock her anyway, which causes her to stabilize. The other band members are frustrated that she seized in the middle of a competition that they were favored to win. They continue to razz her in her room until Karev kicks them out.

The interns found that Patient X has elevated LDH levels, and she quizzes the interns on what to do next.

Callie helps Mark get through the day with some injections, but tells him he'll be in a world of hurt later. Meanwhile, Derek finds out that he's lost more patients than he's saved. Meredith tries to reassure him that many of the patients he lost were terminal before they came to him and that he takes on major cases. Derek says he's killed more than Bundy and quits.

The interns think that Patient X is fine and she yells at them that the patient (Izzie) had hallucinations and is not fine and demanded they keep looking.

During Trish's surgery, they found a tumor and removed it, showing that it was a curative measure. This prompts her sister to change her own mind about the surgery.

Karev goes to Robbins and Hunt about the seizure patient, thinking it might be a cardiac issue and not an epilepsy issue. Owen sides with Karev, although Robbins thinks it's barbaric to shock a young teenager's heart. Robbins puts him in charge of crash cart in case he's wrong about his diagnosis. She has another arrhythmia on the table, and Karev starts to shake with the paddles in his hand. Owen tells him that he's saved her life. They fit her with a pacemaker and she is grateful to Karev.

Izzie reflects on how dedicated George is, and he asks her what is wrong with Patient X. Lexie concludes that there might be something wrong with Patient X's brain. Lexie and the interns conclude that Patient X has metastatic melanoma with mets to the liver, skin, and brain. With chemo and and radiation, the prognosis is a few months. Izzie chastises the interns for asking for a prize. Lexie thanks her for teaching her about diagnosing from beginning to end.

Adele comes to the hospital demanding that Richard and Miranda meet her in the conference room. She mediates their argument and tells her she will not drag herself down to the hospital to police their fights and they better work it out before they leave the room. She then tells Miranda not to call her to "tell on her husband".

Karev runs to Izzie after his victory and tells her that she needs to start working on real patients so they can be great together. He tells her he doesn't want to be the future of the hospital without her.

Cristina tells Owen that it's going to take more than a bad dream to scare her off, and he responds that it's more than a bad dream. The Chief tells Meredith that he needs her to bring Derek back. The Chief tells her that he was planning to propose and that's not a man who wants to be left alone.

Arizona tells Callie that she misjudged the situation and she'd like to take her to dinner. Callie tries to brush her off, but then accepts. Izzie tells Cristina she needs to tell her something because she's a robot and she has to tell somebody, but she can't tell her at the hospital.

Meredith goes to the trailer to talk to Derek, but he drunkenly tells her that she wrote the book on being afraid of commitment and that she's a lemon and there's no fixing her. She tells him the Chief told her there was a ring, and Derek hits the ring into the forest with a bat and tells her to go home.

Cristina takes Izzie to the boiler room and Izzie tells Cristina about her condition.
